Spa Waiver

To use the spa room facilities at the Skyhouse, you are required sign a waiver. The primary contact is the only person who needs to sign, but they accept responsibility for the rest of the group. You can sign it digitally below, or if you email us (stay@unionskyhouse.com) a request for a printable copy, you can sign it by hand and send it via snail mail or email at least one week prior to check-in. Without signing, spa use is not permitted. Please read the waiver & rules thoroughly, and let us know if you have any questions before signing. Complete the form at your earliest convenience.
